If you have made your initial appointment, you only need to bring your medical insurance card, driver's license, glasses and/or contact lens and  medication list if applicable. You do not have any forms to fill out! This will help to speed up the new patient registration process and get you in to see the doctor sooner!

Insurance:
We accept most insurance plans. Please call us to verify whether or not we are participating with a particular insurance company. We offer “same-day-pay” eye health vision exam discounts for those who have no eye-health coverage.

Billing and Financial Responsibility:

We’ll take as much time as you need to make sure you are comfortable and to answer your questions. We also:

• Accept cash, debit cards, Visa, MasterCard, American Express and Discover. Please note that we do not accept checks.
• Are preferred providers with Regence, Uniform Medical, VSP, First Choice, Aetna, United Healthcare, Premera Blue Cross.
• Have an online store so you can conveniently buy contact lenses.
• *We also take some vision plans for the following: Cigna, Lifewise and Group Health.

We gladly accept patients from the Capitol Hill, Broadway Seattle, Belltown, South Lake Union and downtown Seattle areas of Seattle in King County, WA.
